Sustainability and Transformation Plans (STPs)

What is a Sustainability and Transformation Plan (STP)?

Every health and care system in England is now required to produce a Sustainability and Transformation Plan (STP), showing how health and care services will evolve and become sustainable over the next five years and will deliver the aims of the NHS Five Year Forward View  including the Mental Health Forward View

STPs must meet the needs of the local population and drive changes to improve the quality of care, the health and wellbeing of the local population and the efficiency of services. They must also be financially sustainable.

In which STPs is AWP a partner?

Mental health is central to STPs. There are two STPs within the AWP area and we are key partners in both. We are advising both STPs on mental health, highlighting its importance as a 'golden thread' running through the STPs and making sure the plans give mental health parity with physical health.

AWP is one of 17 partners in Bath and North East Somerset, Swindon and Wiltshire (BSW) STP

AWP is one of 13 partners in Bristol, North Somerset and South Gloucestershire (BNSSG) STP that make up Healthier Together

Healthier Together represents a commitment to improving health and care in Bristol, North Somerset and South Gloucestershire. It is about tackling the issues that matter most and finding ways to continue providing safe, high-quality care for generations to come.

We have involved our communities in helping us to shape our plans and will continue to listen and work with local people, staff and the voluntary sector as we progress towards a more integrated approach to delivering health and care in Bristol, North Somerset and South Gloucestershire.

Please visit our dedicated Healthier Together website to find out more: and follow our partnership Twitter feed for latest news: @HTBNSSG.

Please see the BSW plan and BNSSG Healthier Together plan tabs below for full listings of the organisations brought together by each of these STPs.

This joint work will not stop all the great work already going on to improve local services and the STPs will not replace individual organisational plans or wellbeing strategies. But in AWP we will be making sure that our own strategies and plans fit with the STPs and therefore efficient within, supportive of, and supported by our local health and care communities.
