The health and social care records team works closely with the information governance team to safeguard personal and sensitive information held within the health record. All staff are trained in data protection and confidentiality procedures and safeguards.
The Trust has a health and social care records manager who manages the requesting of the health record by AWP staff and the storage, operational movement and transportation of the health record across the Trust.
Also managed are any logistical delays which might occur and the quickest way in which to facilitate the confidential movement of health record information from one point to another as quickly as possible.
Requests to access the health record of a living person under the Data Protection Act 1998 or a deceased person under the Access to Records Act 1990 are managed and centrally logged by the health records team, again working closely with the information governance team.
The health and social care records team are more than happy to liaise with you when requesting access to a record and aim to make the experience as seamless a process as possible.
Any request to access a record can be made to the above address.
