What do you think about community health and care services in Bath & North East Somerset?

A major review of adult and children's community services has begun and your input is needed. The review called your care, your way is being conducted by NHS Bath and North East Somerset Clinical Commissioning Group (CCG) and Bath and North East Somerset Council (B&NES). The review will enable people to research and plan their care and support for themselves.

The CCG and B&NES are reaching out to AWP staff and service users and all residents in the area to hear how existing community services could be improved. This is being done through a number of methods including presentations to patient groups, leaflet drops, media campaigns and online surveys.

The overall aim is to ensure that mental health services integrate as effectively as possible with community health services so that people receive the necessary support to live healthy and independent lives for as long as possible.

With councils and health services all working together, strong links will be formed to make sure everyone gets the care and support they need.
