Specialised Community Forensic Team

The Secure Community Forensic Team (SCFT) is a community-based service providing forensic treatment, monitoring and consultancy to adults who have been discharged from high, medium and low secure inpatient units in the Bristol, North Somerset and South Gloucestershire (BNSSG) area. The service has been commissioned by NHS England/Improvement to provide case management to patients leaving secure care during the transition period.

Based at Fromeside, Blackberry Hill, Bristol, the SCFT is a multi-disciplinary team comprising of Consultant Psychiatrists, Psychologists, Mental Health Nurses, Occupational Therapists, Social Workers, Employment Specialists, an Involvement Coordinator, Peer Support Workers and Administrators.

The team aims to support the patients on their caseload by:

  • Helping individuals who are approaching discharge from secure care to make the transition back to the community;

  • Advising and working collaboratively with other mental health professionals, community mental health teams, GPs and social care staff to support patients with the aim of reducing risk and avoiding the need for re-admission to secure care;

  • Giving expert advice to, and working with, other criminal justice system agencies to manage the risk of patients discharged from secure services into the BNSSG footprint – this is usually via a Multi-Agency Public Protection Arrangement (MAPPA);

  • Providing evidence-based assessment and treatment that will rely on education and training provided by forensic mental health services; and

  • Supporting research and audit within services.

The SCFT holds the responsibility of care co-ordination, social supervision and responsible clinician for the transition period.

The ethos of the service is to embed co-production, and to challenge how it works and engages with service users, their families, friends and carers. To ensure the work is done collaboratively with those involved and that, we listen, hear and take on feedback from those we support.

The service works closely with the mental health charity Second Step to utilise their expertise in helping people into job and educational opportunities. The service also works with Second Step and other third sector housing providers to help develop individual housing that is specifically for the use of patients discharged under the care of the SCFT.
