Intensive Outreach Teams

Our Intensive Outreach Team offer services to young people who are:

• up to the 18 years old 
• living in Bristol or South Gloucestershire
• struggling with complex mental health difficulties
• referred by their locality CAMHS team 

• Each young person will be assigned an outreach worker, a case co-ordinator and a consultant psychiatrist. 
• The Intensive Outreach Team will work closely with young people, their family and the network of professionals.
• The aim of the team is to prevent hospital admission.
• We support children and young people to access and engage with services in their community.
• We work with you to set your own goals and regularly talk to you about how you are getting on.
• Young people and their families will meet with the team multiple times a week for up to three months.

When a child or young person starts working with our team, they will complete a care plan to help them identify why they are using the service, what difference they would like to see, and how the team will know when things have improved.

The Intensive Outreach Team will use this care plan to regularly review the progress of each young person, making sure that the service is helping to meet their mental health needs. 

We follow the Care Programme Approach (CPA) framework which allows young people access to a care coordinator, a care plan, and regular reviews together with their care team, the wider support network and families/carers.

Our team can arrange appointments to see young people and their families at home, school, our office, a café or whilst getting out for a walk somewhere. We will aim to have most of our appointments face-to-face and will offer telephone or video calls to check in if needed.
