The Freedom of Information Act 2000 provides a right of access to a wide range of information held by public authorities, including Avon and Wiltshire Mental Health Partnership (AWP) NHS Trust.

AWP publishes information on this website, and you may well find the answer to your question is already available here before you make a request. If you request information from AWP that is already published, we will help refer you to the published source. 

Who can request information? 

Any member of the public in any location can make a freedom of information (FOI) request to AWP. 

What information can you request? 

Any information recorded or held by the Trust at the time the request is received will be eligible for disclosure. 

Under the Freedom of Information Act 2000, public authorities are not allowed to disclose personal or sensitive information about patients or staff. This information is protected by the Data Protection Act 2018 and the General Data Protection Regulation. 

How can you request information? 

Your request must be made in writing and include details of your name, correspondence address (postal or email), and a description of the information you are requesting.

How we respond to requests 

Once we have received your written request, you will be sent an acknowledgement letter and we will begin processing your request. 

As stated in the Act, we will try to respond to your request within 20 working days.

If you have any questions or are unhappy with how we have dealt with your request, please write to the above address and we will conduct an internal review. 

Copies of every request we receive will be held on file for three years before being confidentially destroyed. We may also publish your request within our Freedom of Information Request Log. All personal data will be removed prior to publishing.
