What is Your Team. Your Conversation. Your Plan?

This is a simple, flexible and personalised way of supporting your mental health and
wellbeing. We will work in partnership with you and the people who support you.
Together we will plan care and support that helps you to achieve the goals that are
important to you.

Your Team.

Your team will work to support your wellbeing. You are at the centre of your team. People
that are important to you can join. You may wish to include your:

  • Support Worker;
  • Family, carers or friends;
  • Nurse;
  • Support from another organisation;
  • Psychologist;
  • Social Worker, etc.

The Keyworker

A member of your team will be your keyworker. You will help to decide who your keyworker is.

Your keyworker will be:

  • The main point of contact for you and your team;
  • Best placed to support you;
  • Someone that you have a good relationship with;
  • A member of any organisation involved in the delivery of mental health support;
  • Someone who has access to our records system and works closely with other members of your team.

Your conversation.

You will have regular Support Conversations. These will help find out your goals for
recovery, personal strengths and needs. You and your team will talk about anything
that is important to you. For example these may include:

  • Physical and mental health;
  • Housing;
  • Relationships;
  • Employment;
  • Things you enjoy;
  • Money.

We will also help you to think about safety during support conversations. Safety is protection from anything that causes harm and it is important to understand your views
on this.

Your plan.

In your Support Conversations, we will work together to develop a Personal Wellbeing
Plan (PWP). This is your plan and will include a clear and simple outline of:

  • What is important to you;
  • What you would like to achieve;
  • The support that you need;
  • Things that keep you safe.

You will always be given a copy of your plan.

Consent to share

Sharing relevant information with your family, carers, friends and other organisations may help to achieve your aims. It is important for us to understand what you would like to share, and this can be discussed at any point. For more information about this, visit the
